Description:	Hemoglobin A1C is a form of glycated hemoglobin. With the worldwide explosion of the diabetes epidemic, it was welcome news in 2010 that the A1C assay had been standardized and that hemoglobinopathies such as sickle cell trait did not interfere with A1C interpretation. Therefore the American Diabetes Association and the International Diabetes Federation now recommend A1C as a test to determine glucose tolerance status including pre-diabetes and diabetes. In this Director’s Lecture, data will be presented which contradicts the view that A1C is an appropriate test to use in Africans with sickle cell trait. As the prevalence of sickle cell trait in equatorial Africa is 10 to 40%, we will suggest that even though the momentum to use A1C to diagnose glucose tolerance is rapidly growing, more research is necessary before A1C is used to determine prevalence of either pre-diabetes or diabetes. We will also present our findings on glycated albumin which is a red blood cell independent marker of glycemic status and therefore unaffected by sickle cell trait. Glycated albumin has the potential to provide a more reliable path to the diagnosis of diabetes, and guide the allocation of public health resources in populations where sickle cell trait is highly prevalent.
